Personal applications?  These are what I know:

Chaos Mages, Chaotes, Chaotists, Chaoists -- typically applied by those who
			have had some association to Peter Carroll and his ilk

Chayoat -- applied by me to those who pretend to be chaotic and are really
	   more of the same, perhaps Satanists in disguise

Kaote -- my own style of chaos, without definition or constriction

I can't think of any others I've seen/made-up.  I could make up more if you
like, however. :>

Those UNCLASSIFIABLES WHO NEED NO LABEL?  I call those 'Kaotes', personally,
and promulgate the true and ugly falsehood of the One Kaotik Truth (that is
that there is no One Truth), the principle of Undefinition (definition
destroys Kaos), and the essential Rule of Rulelessness (that the term
'chaotic organization' is an oxymoron).  :>
